Custom vs Off-the-Shelf Orthoses: What’s Right for Your Feet?

When it comes to foot pain, discomfort, or ongoing biomechanical issues, orthoses (or orthotics) are often recommended to support your feet and correct alignment.  But not all orthoses are created equal.  At Dulwich Podiatry, we’re often asked whether custom-made orthoses are worth the investment compared to off-the-shelf alternatives.  The short answer?  It depends on your feet, your lifestyle and your long-term goals.

Off-the-Shelf Orthoses: A Quick Fix

Off-the-shelf orthoses are pre-manufactured insoles that you can buy from pharmacies, sports shops or online.  They come in standard sizes and offer general arch support and cushioning.

They can be helpful if you:

  • Have mild discomfort or fatigue after standing all day
  • Need temporary support during travel or work
  • Are looking for a low-cost short-term solution

However, they’re not tailored to your foot structure, gait or pressure points.  While they may help in the short term, they rarely address the underlying causes of pain or instability.  At Dulwich Podiatry, we issue off-the-shelf orthoses where generic support is appropriate to your foot condition or as a temporary solution while waiting for your custom orthoses.

Custom Orthoses: Personalised Support for Lasting Results

Custom orthoses, on the other hand, are designed specifically for your feet.  At Dulwich Podiatry, we carry out a thorough biomechanical assessment and gait analysis before prescribing orthoses.  These devices are made using 3D scans or casts to precisely fit the shape of your feet, offering tailored support where you need it most.

They are ideal if you:

  • Suffer from conditions such as plantar fasciitis, flat feet, bunions or arthritis
  • Have complex or longstanding foot pain
  • Need orthotic support integrated into daily, work or sports footwear
  • Want a long-term, preventative solution

Here’s a helpful analogy: orthoses are a bit like glasses. You can pick up a pair of generic reading glasses from the shop, and they might work well enough for occasional use. But if you have a specific prescription or eye condition, only tailored lenses will give you clarity and comfort.

The same goes for your feet. A generic insole may cushion your step, but only a custom orthosis can correct the way your foot moves, distributes weight and responds to pressure over time.
